You will be a member of the neuro rehab team/early supported discharge stroke service and will be responsible for the assessments, planning, implementation and evaluation of individualised speech & language therapy intervention to patients living in the community.
You will be based in the community working a range of shifts patterns to meet the needs of our patients.
As a Bank worker you can work flexibly full or part time to fill a temporary vacancy or short term staffing shortages, however hours and placements cannot be guaranteed.
Our temporary roles will give you lots of variety in different environments and could be your “foot in the door” to the NHS.
To be responsible for coordinating intervention for designated clients acting as their Case Manager whilst they are receiving input from service.
To contribute to service development and project leadership.
To undertake lone working in the community.
To work a variety of shift patterns which include weekends and bank holidays on a rota basis.
Management and provision of highly specialist Neurology Speech & Language Therapy , using models of best practice to assess the client’s needs, and to provide therapy, advice and support for the patient.
Evaluation of clients’ needs in order to give effective support at the right time, involving prioritisation of the caseload, according to recently reviewed literature and clinical effectiveness guidelines.
Management and provision of highly specialist SLT for specified clients, by assessing needs and then providing programmes and resources, which are reviewed and up-dated regularly.
Support for colleagues within and outside of Speech & Language Therapy, including the provision of student placements.
Support and engage in the development of the stroke early supported discharge service.

CLINICAL
To assess, develop and implement Highly Specialist Speech & Language Therapy treatment and evaluate outcomes.
To use highly specialist skills to formulate a diagnosis.
To provide highly specialised written and verbal advice to a range of professionals regarding the management and care of clients with severe/ complex communication and/or feeding and swallowing difficulties.
To maintain highly specialist knowledge of current practice and literature, ensuring decisions are based on the best available evidence.
To adapt clinical practice to meet individual patients’ circumstances, including due regard for cultural and linguistic differences.
To be responsible for the security, care and maintenance of equipment ensuring standards of infection control and safety are maintained – including equipment loaned to clients.
To communicate complex condition related information from assessment to clients, carers, families and members of the multi-disciplinary team/other professions.
To contribute to clinical teams both uni-disciplinary and multidisciplinary by discussing own and others input around clients’ needs ensuring a well-coordinated care plan.
To demonstrate empathy with clients, carers and families and colleagues, ensuring that effective communication is achieved, particularly where barriers to understanding exist.
To demonstrate skills in motivating clients with severe communication needs and /or carers to engage in the therapeutic process and enabling them to contribute to the decision making process regarding their care plans.

To demonstrate the ability to reflect on auditory, visual and kinaesthetic aspects of client’s communication and to identify appropriate strategies to facilitate and enhance communicative effectiveness.

CASELOAD MANAGEMENT
To manage and prioritise complex and specialist caseload and workload independently and use specialist knowledge to inform sound clinical judgement/ decision making.
To use highly specialist knowledge to develop protocols and care packages in areas of expertise.
To liaise with relevant user groups as appropriate.
To monitor and evaluate own highly specialist service delivery.
To produce reports reflecting highly specialist knowledge for individual clients and client groups.
To be an active member of the multi-disciplinary team, contributing to service delivery, audit and development.
To provide highly specialist advice/ second opinions to speech and language therapists and other professionals in areas of expertise.
To maintain up to date and accurate case notes in line with RCSLT professional standards and Trust policies.

To gather activity data accurately and regularly, ensuring the provision of such information promptly within trust guidelines.
To demonstrate skills in dealing with complex issues to generate appropriate strategies for caseload management.
To develop care protocols for specific clinical groups in liaison with the multi-disciplinary team and Team Leader to improve patient care.
To advise Team Leader of issues of service delivery including shortfall, service pressures and to use highly specialist knowledge to inform service/ policy developments as appropriate.
To maintain intense concentration in all aspects of patient management for prolonged periods.

To assist in the recruitment and selection of staff for the service and be responsible for the induction and orientation programmes as appropriate.
Assist in management of conflict and tensions that may arise within the team.
To participate in the Individual Performance and Development Review process and carry out reviews of junior staff in consultation with the Team Leader.
To participate in the Clinical Supervision programme as supervisor and supervisee.
To deputise for Team Leader and assume delegated responsibilities as appropriate.
To participate in student placements within the service and to maintain training as student fieldwork educator.
To provide education, advice and training as required to the Multi-Disciplinary Team.
Be responsible, in conjunction with the Team Leader, for implementing the appraisal process for development of staff.
Supervision of junior staff and students.
To provide advice, teaching and instruction to unqualified staff, relatives, carers and other professionals to promote the understanding of the aims of occupational therapy, rehabilitation and a constant approach to patient care.
Maintaining own professional development and requirement to take part in appraisal and KSF process.
INFORMATION RESOURCES
Complete and maintain all documentation in a contemporaneous style, legibly, accurately and in a concise manner. This will include records of patient visits, profession specific paperwork, referral documentation, outcome measures and patient screening tools.
To maintain timely and accurate patient records, which include patient held records.
To record and report accurate and timely data in relation to all contacts with clients and other activities onto the computer system within 24 hours.
RESEARCH & DEVELOPMENT
To contribute to meetings about clinical governance and service development.
Undertake audits in line with Trust policy and clinical governance agenda.
Undertake or participate in research in order to improve service as required.
To participate in the delivery of the Rehabilitation and Enablement Team development plan.
